Objectives: This study aimed to investigate the association between ultra-processed food intake and colorectal adenoma among Korean adults.

Methods: A total of 599 participants aged over 20 years, who underwent gastrointestinal endoscopy across eight medical centers from 2021 to 2023, were included in the study. Participants diagnosed with colorectal cancer, inflammatory bowel disease, proctitis or hamartomatous polyps were excluded. The daily intake of ultra-processed food by the participants was estimated as the percentage of energy derived from ultra-processed food in their total diet, using a structured food frequency questionnaire. We excluded alcohol intake from the ultra-processed food consumption. Multivariate logistic regression models were used to assess the association between ultra-processed food intake and the risk of colorectal adenoma, calculating odds ratios (ORs) and 95% confidence intervals (CI).

Results: Out of 599 study participants, 262 were diagnosed with colorectal adenoma (167 men and 95 women). We did not find a significant association between ultra-processed food intake and colorectal adenoma in either men or women; ORs (95% CIs) for subsequent quartiles compared to the first quartile were 1.32 (0.63-2.77), 1.72 (0.81-3.65) and 0.74 (0.34-1.62) for men, and 0.55 (0.25-1.23), 0.67 (0.29-1.53), and 0.68 (0.29-1.62) for women.

Conclusions: Ultra-processed food intake was not associated with high prevalence of colorectal adenoma in Korean adults in this study. Further investigation is required to explore the association between ultra-processed food intake and the risk of colorectal cancer precursors in Asian populations.
