(LB02-04-24) Sleep Duration Modifies the Association of Alternate Mediterranean Diet Score With Metabolic Syndrome in Mid-Life Women in Mexico

Objectives: Adherence to a Mediterranean diet and better sleep health have each been associated with a lower risk of metabolic syndrome (MetS), but the potential effect modification of sleep is not well understood. This cross-sectional study examines whether sleep duration modifies the association between adherence to the alternate Mediterranean (aMed) diet and MetS among mid-life Mexican women.

Methods: The analytic sample comprised 410 women with a mean age of 48.23±6.15 years participating in the Early Life Exposure in Mexico to Environmental Toxicants (ELEMENT) study. Diet was assessed with a validated food frequency questionnaire, and adherence to the aMed diet was evaluated based on consumption of 8 food groups. The mean daily sleep duration was measured with 7-day wrist actigraphy (Actigraph GTX-BT). MetS was defined based on the American Heart Association (AHA) criteria. Multivariable logistic regression models were employed to estimate the association of aMed diet with MetS, controlling for age, socioeconomic status, and moderate-to-vigorous physical activity (MVPA). To assess the potential modifying role of sleep duration, we conducted stratified analyses based on sleep status, where participants were categorized as having adequate sleep (≥ 7 hours/day) or inadequate sleep ( < 7 hours/day).

Results: MetS prevalence was 49.8%, while 43.4% of participants slept < 7 hours/day on average. Higher adherence to the aMed diet was not associated with MetS in the full sample. Among women with inadequate sleep duration, however, aMed diet was related to a 16% reduction in odds of MetS (OR: 0.84, 95% CI: 0.70-0.99).

Conclusions: A Mediterranean diet may be especially important in preventing MetS among women who are short sleepers.
